Skin assessment in ED and within 2-6 hours of admission to
the acute trust and document in the Skin Bundle/Pressure
Ulcer Daily Care Plan.
Identify any skin changes
Identify and grade any existing or new pressure ulcers
using European Pressure Ulcer Advisory
Panel (EPUAP) classification tool
Identify wounds which are not pressure related
On-going skin checks when repositioning and document any
new changes
