Pressure Ulcer Risk Assessment
Initial Pressure ulcer risk assessment within 2-6 hours using a
designated risk assessment tool such as Waterlow.
Regular, frequent review of the pressure ulcer risk throughout
the in-patient care episode with additional review if;
The patients condition changes
The patient returns from any care intervention outside of their
clinical area (Surgery, Radiology)
When the patient is transferred into a new care setting
