Inform the Dr that your patient has fallen (including if the patient is on any
anti-coagulants, if they banged their head, how the patient was found indicating
how they fell)
Complete a Datix Incident Form - No fall should be classified as a near miss
Harm levels are defined as
No Harm
Low Harm Bruise or laceration
Moderate Harm Laceration requiring surgical closure, any fracture
Inform the relatives
Document the incident and all communication in the patients medical notes (not
end of bed nursing notes)
Review the care plan and rewrite as necessary
Complete the required schedule of observations clearly marking them as post
falls observations
