Nutrition Screening
  • This is an assessment which identifies patients who are
  • malnourished or at risk of malnutrition

  • The CQC requires all patients to be nutritionally screened within 24
  • hours of admission
    •  
  • Screening should then be repeated weekly or if the patient's
  • condition changes.
    •  
  • Always check the score when a patient transfers from another ward
  • and repeat if an assessment has not been carried out in the last 7
  • days
    •  
  • The score will indicate what actions are required to be carried out
  • by nurses and other health care professionals. An appropriate
  • Nutrition Care Plan should be made based on these.
