Commence BH Adult SSKIN Bundle Pressure Ulcer Care Plan
on patients with a Waterlow of 10 or more
Re-assess pressure ulcer risk if there is a change in clinical
status- e.g. after surgery, worsening of an underlying condition
or a change in mobility and on transfer to your area
If patient inititally assessed as no risk reassess Waterlow score
weekly
