When discharging any patient with an existing pressure ulcer or
who is at risk of developing a pressure ulcer consider the following:
Complete a body mapping on day of discharge
Pressure Relieving Equipment (Mattress, Chair, Chair cushion etc)
Moving and Handling needs at home (Profiling bed frame, hoist,
slide sheets)
Repositioning including periods of bed rest, restricted sitting,
standing and mobilising and how this is to be achieved ie: Does
the patient need a care package
Nutritional support
District Nursing for on-going pressure ulcer risk assessment and/or
wound management
Referrals to other specialist services or provide equipment
