Discharge planning
When discharging any patient with an existing pressure ulcer or
who is at risk of developing a pressure ulcer consider the following:
 
  • Complete a body mapping on day of discharge
  • Pressure Relieving Equipment (Mattress, Chair, Chair cushion etc) 
  • Moving and Handling needs at home (Profiling bed frame, hoist,
  • slide sheets)
  • Repositioning including periods of bed rest, restricted sitting,
  • standing and mobilising and how this is to be achieved ie: Does
  • the patient need a care package 
  • Nutritional support
  • District Nursing for on-going pressure ulcer risk assessment and/or
  • wound management 
  • Referrals to other specialist services or provide equipment
